Limited Confidentiality Clause
"I acknowledge that the information contained herein is confidential and, pursuant to the Privacy Act, will only be strictly used by the Ngunguru After School Care team to effectively care for my child and not used for any other purposes. Representatives from the social services procurement team, Ministry of Social Development may view this information as part of the program assessment process". Information of children held by Ngunguru After School Care will not be made available to third parties and will only be accessible to authorized staff members. At any time, parents/caregivers may request to see their child(ren)’s information
Children must be collected from Ngunguru After School Care no later than 5:30pm each day.
Upon collection, parents/caregivers must sign your child/children out.
A late fee of $25 per fifteen minutes, or part thereof, from 5:30pm will be charged.
- COLLECTION OF CHILDREN BY SOMEONE OTHER THAN THE PARENT/CAREGIVER
Parents/caregivers must advise ASC if their child(ren) are being picked up from ASC by someone other than themselves prior to pick up.
Fees are payable within 7 days of Invoices being issued.
- DEFAULT ON ACCOUNT
ASC reserves the right to charge interest on overdue invoices. Unless a payment plan has been negotiated with the Manager,
ASC reserves the right to exclude from the Programme children whose behaviour prejudices the safety or health of others.
ASC shall be advised of any change in contact details for the parents/caregivers of the children.
ASC shall be notified of any custody issues relating to the child(ren) with necessary documentation.
If necessary, the Manager may arrange for urgent medical attention for the child(ren), the cost of which is to be met by the Parents/Caregivers.
BINDING AGREEMENT
a) This is a binding agreement between Ngunguru After School Care and the parents or guardians of the child(ren) enrolled.
By enrolling your child you have read and understood the programme and the information provided and hereby agrees to the Terms and Conditions detailed above.
IMAGES
Images of our ASC students (photographs, video clips, etc), and examples of their work, are sometimes published in our newsletters, or on our school website.
We publish student material to celebrate students' work and achievement, to encourage students to participate in our school community, and to promote the school.
- I give permission for my child’s first name, image, or work to be published in the After School Care newsletter, on the school website.
